Injection site rotation is the least glamorous habit in GLP-1 treatment and one of the easiest to get wrong — not because it's difficult, but because it depends entirely on remembering what you did seven days ago.
Why rotate at all
Repeatedly injecting the same patch of skin can leave it tender, bruised, or thickened over time. Rotating gives each area weeks to recover, keeps injections comfortable, and avoids building up problem spots you then have to work around for months.
For weekly GLP-1 medications, the approved sites are typically the abdomen (avoiding the area immediately around the navel), the front of the thigh, and the back of the upper arm. Check your own medication's instructions — they are the authority, not this article.
The 8-zone system
Treat your body as eight zones — left and right of each site — and move one step each week:
- Left abdomen
- Right abdomen
- Left thigh
- Right thigh
- Left upper arm
- Right upper arm
- Left flank
- Right flank
Cycle in order and any given zone gets roughly two months to recover before you return to it. That's comfortably more than enough.
Three rules that prevent most problems
- Never the same spot twice in a row. Different zone beats different spot in the same zone.
- Skip compromised skin. Bruised, hard, scarred, tender, or irritated — move on and come back later.
- Write it down at the time. Not later. You will not remember.
The remembering problem
Every system above fails on the same step: recalling, a week later, which zone you used. A notebook works until you forget it once, and then the sequence is broken.
